Important:

• This application requires a screening process and any updates / status of an application will be communicated to you accordingly via email.
• Kindly ensure the applicant’s details are reflected in the Annual Practicing Certificate (APC) and X-ray license.

The process for this application may take up to four (4) weeks. The required documents for the screening process are:

1. A copy of NRIC (both sides).
2. A copy of latest APC.
3. Evidence of registration with the Ministy of Health Malaysia (MOH) under the Private Healthcare Facilities and Service Act 1998 and Private Medical Clinics (Borang B/F).
4. A copy of X-ray license along with Lampiran A
