  2. Original passport
  3. Copy of original passport
  4. Copy of working permit/ RTK approval slip/ SMO approval slip
  5. Payment MYR207 male/ MYR217 female via bank draft / CIMB clicks / QR Pay (DuitNow)
  6. Authorisation letter for representative only
  7. Copy of NRIC of employer/ representative

Employer is highly advised to cross the top of your personal document with “For FOMEMA use only” to protect your personal data.
