Important:
- This application requires a screening process and any updates / status of an application will be communicated to you accordingly via email.
- Kindly ensure the applicant’s details are reflected in the Annual Practicing Certificate (APC).
The process for this application may take up to four (4) weeks. The required documents for the screening process are:
- A copy of NRIC (both sides).
- A copy of latest APC.
- Evidence of registration with the Ministy of Health Malaysia (MOH) under the Private Healthcare Facilities and Service Act 1998 and Private Medical Clinics (Borang B/F).
